Located at Union Chapel Church on the northeast side of Chapel Creek Road about two miles west of CR 223 (Dry Run Road). It is in Section 36, just north of the Springfield Township line. The church has over its door a sign "Built 1851 - Rebuilt 1903." The cemetery was copied in 1970 by Marie Clark. It was copied again in November 1998 by Bob Casari and 2008 by Johnda Scott and Herman Gray.

The rows were numbered from the northwest (front of the church) to the rear of the cemetery beyond the rear wall of the church. Rows were read from the southwest end nearest the church building.
